At St Andrew’s Hospice, our mission is to make each day count. As a registered charity, we provide palliative care services for adults, children and families who are living with life-limiting conditions, as well as family bereavement support.

As part of the healthcare charity, The Healthcare Management Trust, HMT St Hugh’s Hospital is a not-for-profit organisation. As such, we have no shareholders and our surpluses are used to support dementia charities, educational grants, and medical research around ageing.

We provide a gateway and offer a smooth and structured transition from a range of services back into the community.

Sussex House Care Home prides itself on quality person-centred care. We focus on promoting independence through a range of support services for those who have mental health difficulties, as well as a combination of mental health and other needs such as physical disabilities or recovery from alcohol/substance misuse.

Sussex House is only one of two residential homes commissioned for Mental Health by the North East Lincolnshire Commissioning Group.

Friendship at Home CIO is a registered charity that began in September 2006 initially funded by a legacy left to Help the Aged, when a senior member spent the day with the local Handy Van and witnessed first-hand the loneliness and isolation that older people faced on a daily basis and wanted to make a commitment to help. The scheme initially only covered the Cleethorpes area but it soon became evident that more needed to be done.

Friendship at Home now offers friendship and support to older people, covering the whole of North East Lincolnshire.

Lifeways is the UK’s leading team of support professionals for adults with diverse and complex needs living fulfilling and independent lives in the community.

We support people living with any or a combination of the following:

Humbercare was established to work with the Humberside Probation Trust. We work to: educate, rehabilitate and promote the mental and moral improvement of offenders, or any other persons in need and the rehabilitation of persons discharged from penal institutions. Humbercare received charitable status during 1989 and became a company limited by guarantee in October of that year. Humbercare came from a desire to be able to offer additional help and assistance to offenders and those at risk to reduce the risk of re-offending through local projects.

Humbercare is an enabling organisation dedicated to providing quality and innovative services to adults and young people in need. The Project will deliver needs appropriate, face to face, housing support for Young People, and those with a Physical Disability or Learning Disability in North East Lincolnshire.

YMCA Humber is a youth charity that supports anyone over 16 years old who is at risk of homelessness, living in North East Lincolnshire. We specialise in supported accommodation for young people by providing them with a safe environment which they can call a home, enabling individuals to grow and achieve their full potential. Alongside providing accommodation we also offer a range of health and wellbeing drop-in sessions, youth and family services, a community café, and conference facilities.

Set in the peaceful market town of Waltham near Grimsby, Ashlea Court is a purpose built, 47 bed residential care facility for older people providing care on both a short term and permanent basis.

For over 18 years Willow Homecare & Support Services Limited have been delivering specialist and person centred care for a variety of service users over the age of eighteen inclusive of both elderly and people with a learning disability. The Roxton Practice has provided medical services to the people of Immingham and the surrounding area for more than 60 years. The first home for the practice in Immingham was a small house in Manby Road, serving the population of Immingham.

Eastwood House Residential Care Home, located in Grimsby, is a small and supportive facility providing care for up to 19 residents. It specialises in caring for older people and individuals living with mild to moderate dementia, offering both permanent and respite residential care. The home features 17 single rooms and one companion room, with some rooms including ensuite wet rooms. Designed for comfort and accessibility, the home includes facilities such as a lift, stairlift, wheelchair access, gardens, and TV points in rooms. Eastwood House is owned by Forthmeadow Ltd and managed by Stacey Riggs. It is registered and regulated by the Care Quality Commission (CQC) and is committed to delivering a safe, respectful, and community-focused environment tailored to the needs of its residents.

Our practice is made up of 3 purpose-built sites, that are run by 4 GP Partners, 6 Salaried GPs and 2 Managers and are supported by a large, experienced team of clinicians, administrators and receptionists. We care for a population of approximately 24,000, in the Grimsby and Cleethorpes areas.
